You may also have vision coverage through a separate policy that might provide partial coverage for vision care, like eye exams and glasses. If you are not familiar with the specifics of your insurance benefits, please review them to better understand the benefits that are part of your insurance coverage. The patient payment portion covered for procedures varies depending on the coverage. Copays and deductibles are due at time of service. If you are unsure of these amounts, please contact your insurance company prior to your visit.
